Children's Transitional Home:
Dooley House is a HOME for medically fragile children.
The children range in age from one-day to 12 years old.
The house is open 24 hours a day, 365 days a year.
In this warm, loving atmosphere, the children have
access to medical care through an on-all physician,
nurses on site and health care workers who attend the
children at all times. The staff strives to keep the
children healthy, happy and developmentally stimulated. Hogan House:
Hogan House is a transitional residence for
persons and families affected by HIV/ AIDS. This facility
provides housing for homeless and/or displaced persons living
with HIV. Hogan House provides staff and
programs that instruct, demonstrate, and assist residents.
Through individual case management, residents will develop life skills,
have access to job
training, addiction services and linkages to mental health
services and medical care. The goal is to transition each
resident back into the community better prepared to live with
an HIV diagnosis while leading a productive life.
Case Management: The case
managers, in collaboration with community agencies, help
HIV/AIDS individuals develop support systems. Plans of
Care are developed and implemented for each client. The
plans include in-put from clients, along with medical and social
agencies. The plan is to provide the beginnings of a Road
Map to independence and a path back to community living.
DH Transportation:
Covering Burlington, Camden, Gloucester and Salem counties, the
transportation program provides access to medical appointments.
The program has specially equipped vans to meet the needs of the
served population. The program provides clients with
dependable, reliable, and a safe transportation source to their
medical appointments.
RW Cares: Recently established
as an entity of Dooley House, RW Cares coordinates
transportation for persons living with HIV/AIDS in Burlington,
Camden, Gloucester and Salem Counties. Transportation
services are provided for medical, dental, support and social
service appointments in the four counties as well as
Philadelphia, In addition, RW Cares coordinates case
management services in Southern New Jersey, providing training
and education for case management agencies in the four counties. DH
Supportive Permanent Housing: The Supportive
Permanent Housing program is new to the Dooley House family of
services. The goal is to assist individuals with mental
health, HIV/AIDS and substance abuse issues to obtain stable
housing. These individuals are offered support services
from case managers, peer mentors and mental health providers.
Vocation, occupational and life-skill training is also provided.
The goal is to maximize each individual's potential through
stable housing and supportive services.
